After Shibuya is destroyed and supernatural anomalies spread, grotesque entities called Aberrations roam the streets.
One protagonist, Souma Hoshigami, is a delivery worker who is attacked by an Aberration and transformed into a 'Half-turned' state where human and monster coexist.
The other protagonist, Rena Hizuki, is a lethal investigator assigned to the Tokyo Metropolitan Police Department's Public Safety Division, specifically the Foreign Affairs Fifth Division, also referred to as the Public Safety Mutation Response Division.
Players will alternate between Rena and Souma to pursue the causes behind a demonic narcotic known as Tiny True, body-carved glyphs called Stigmas, and the cult operating behind the catastrophe.
